Das Urbild dieser Ikone befand sich in der Photinoskapelle der Blachernenkirche zu Konstantinopel, weshalb diese Darstellungen der Gottesmutter oft auch den Beinamen "Blachernitissa" tragen. Es war ein Marmorrelief der Gottesmutter in Orantenhaltung, mit dem Jesuskind in einem Rundschild auf der Brust. Aus den Händen der Muttergottes floß Wasser in ein Marmorbecken. Diese Blachernitissa ist oft auf Ikonen gemalt worden. Auch nach Russland gelangte diese Form des Muttergottesbildes. Die bekannteste Ikone ist hier ein Gnadenbild in Nowgorod gewesen, welches nach folgender Begebenheit sehr berühmt wurde: Als im Jahre 1170 der Wladimir-Suzdaler Fürst Andrej Bogolubskij die Stadt belagerte, hatte der Erzbischof der Stadt, Johann, eine Vision, in welcher ihm befohlen wurde, mit der in der Kathedrale aufbewahrten Ikone der Gottesmutter eine Prozession um die Stadtmauer zu unternehmen und dann das Bild auf die Mauer zu stellen.
